Homeowner SMS Consent Form

Last Updated: February 4, 2026

1. Purpose

This page provides the public, standardized SMS consent disclosure language used by contractors when collecting homeowner consent for messaging through HailMate. Consent is collected in person — during neighborhood storm-damage assessments, roof inspections, and estimate requests — and by phone, with the disclosure below presented verbally at the time of opt-in. Contractors may also present it on a web form where one is used. In every case it is provided prior to sending any SMS/MMS messages.

2. Consent Form Template

By providing your phone number, you agree to receive SMS/MMS messages from [Contractor Business Name] via HailMate regarding estimates, scheduling, project updates, and customer care. Message frequency varies. Message and data rates may apply. Reply HELP for help and STOP to opt-out. Consent is not a condition of purchase.
